Policy Updates

The first responsibility of Guadalupe Clinic to patients is to provide safe care. The policy below addresses the various options for management of pain. Its purpose is to also protect providers and patients from dealing with very addictive narcotics including opioids and other addictive drugs such as Benzodiazepines.

At Guadalupe Clinic, our priority is to provide timely and comprehensive healthcare services to all patients. However, missed appointments without prior notice disrupt our schedule and prevent us from accommodating other patients in need of medical attention. To ensure efficient use of our medical resources we have implemented the no show policy below.

Primary Care Services

Guadalupe Clinic offers a range of primary care services to our patients, including: physician visits, diagnostic tests and imaging, women's wellness, early detection works, diabetic management, pregnancy tests, pharmacy consultations, behavioral counseling, TB tests, seasonal vaccines, and COVID-19 testing and vaccines. 

 

Sliding Fee Scale Policy and Procedure

Guadalupe Clinic does not have a sliding fee scale. We are a free safety-net clinic. Therefore, our patients receive all services free of any charges.

 

Please note: Guadalupe Clinic is a 501(c)(3) private health clinic serving adults 18 – 84 who fall within 200% of the federal poverty guidelines and do not have insurance of any kind. Guadalupe Clinic does not and shall not discriminate based on race, color, religion (creed), gender, gender identification, age, national origin (ancestry), disability, marital status, sexual orientation, or military status.

Specialty services offered to Guadalupe patients include: cardiology, dermatology, ENT, gastroenterology, gynecology, optometry, orthopedics, physical therapy/physiatry, podiatry, pulmonology, and urology.

Required documentation to bring to your appointment:

  • Photo ID

  • Proof of income

 

Patients must provide one of the following documents:

  • Last year's federal tax return (IRS Form 1040);

  • Wage and Income Transcript (IRS Form 4506-T);

  • Low Income Energy Assistance Program (LIEAP) benefit amount/approval letter; or

  • Food Assistance Program ("food stamps") benefit amount/approval letter

 

Attention: Patients must submit required documentation to staff to verify eligibility for clinic services and medication assistance. Required documentation must be submitted by next office visit or medication refill request. Failure to do so may result in loss of services/assistance.

COVID-19 Services

Guadalupe Clinic no longer carries or offers COVID-19 vaccines. The Sedgwick County Health Department will continue to provide them for anyone needing vaccinated. Click here for more information.

Guadalupe Clinic will continue to provide FREE Antigen (Rapid nasal swab) COVID tests at the South St. Francis clinic ONLY. No appointment necessary. Please be patient.
